Need to file a phone protection claim? Here’s how to get started.

Step 1: Call the benefit administrator

Dial 1-866-894-8569 to report the damage or theft.

Step 2: Share your info

You’ll be asked for basic details about your phone and what happened. They’ll send you the right claim form to complete.

Step 3: Ask questions

Important reminders:

  • Report your claim within 60 days
    Claims must be reported within 60 days of the damage or theft. Late claims may be denied.

  • File a police report for stolen phones
    If your phone was stolen, file a police report within 48 hours. A copy is required for your claim.

  • If the cell phone is damaged
    Do not discard it until the claim has been fully reviewed.
